The Peaceful Living Plan
Changing the Conversation About Long-Term Care and End of Life Planning
Life is full of beautiful moments, but it also brings seasons of change. As we move into the next stage of life, many of us begin thinking more deeply about our health, our independence, and the legacy we will leave for the people we love.
The Peaceful Living Plan is a thoughtful, compassionate approach to long-term care and end-of-life planning that helps individuals and families move beyond difficult conversations with clarity and confidence. By focusing on personal values, open communication, and proactive decision-making, we empower you to create plans that honor their wishes while easing the emotional burden on loved ones. The goal is simple: to replace uncertainty and stress with peace of mind, dignity, and a sense of control over life’s most important moments.
The Peaceful Living Plan was developed as a collaborative service designed to help individuals and families approach the future with confidence, clarity, and compassion. This unique program brings together a shared passion for helping people live better lives as they age. A dedicated team of professionals guides the Peaceful Living Plan: Bridget M. Walker, an experienced Estate Planning and Elder Law Attorney; Rachel Nash, a Board-Certified Patient Advocate; and Tara Lee, a Registered Nurse and End-of-Life Doula.
Together, they bring a unique blend of legal, advocacy, and end-of-life care expertise, working collaboratively to support individuals and families with compassion, clarity, and a holistic approach to planning for the second half of life.

The Peaceful Living Plan includes a comprehensive estate plan to protect and preserve what matters most; but it also addresses the deeply personal questions families face as they age, questions about care, dignity, independence, and how we want to be supported throughout life’s later chapters.
Components of The Peaceful Living Plan:
- A comprehensive estate plan includes, but is not limited to, a Revocable Trust, Pour Over Will, Durable Power of Attorney, Health Care Power of Attorney, and Living Will.
- A detailed written long-term care plan that prepares your family for future care needs and makes your final wishes known
- A compassionate end-of-life plan that clearly communicates your wishes and values
- Guidance on funding your long-term care plan, including practical strategies to make sure you can cover costs when the time comes.
- Caregiver Trust – Revocable or Irrevocable – the legal document ensuring your long-term care wishes are honored and the detailed plan for how to pay for care.
- Bi-annual Review to keep your plan aligned with life’s changes
The Peaceful Living Plan is designed for those who want to stay in control of their choices, maintain flexibility as life evolves, and leave a clear, thoughtful roadmap for their loved ones to follow. It’s for individuals who value preparation over uncertainty—choosing peace over chaos—so that, no matter what the future holds, their wishes are known. Their families are supported with clarity and confidence.
